Notification Of Mailing Address – JV-140
Parents or Guardians of the minor(s) are required to provide their mailing address to the court to receive notices or documents. The form must also be filed with the court when a change is made to their mailing address, residential address or a confidential address.
Request For Disclosure Of Juvenile Case File – JV-570
This form is used whenever a person other than the minor or parent/guardian is requesting to inspect or obtain copies from a Juvenile Court file.
Petition To Obtain Report Of Law Enforcement Agency/Juvenile – JV-575
This form is used whenever a person is requesting a police report involving a minor or a minor that was taken into custody. Notice Of Intent To File Writ Petition And Request For Record – JV-820
The Notice of Intent is used if the juvenile court has decided it will make a permanent plan for your child that may result in the termination of parental rights and adoption of the child. If you want an appeals court to review the juvenile court’s decision, you must first tell the juvenile court by filing a Notice of Intent. You may use this form as your Notice of Intent. In most cases, you have only 7 days from the court’s decision to file a Notice of Intent.
Notice To Appeal (Juvenile) – JV-800
California Rules of Court 8.406 says that to appeal from an order or judgment, you must file a written a notice of appeal within 60 days after rendition of the judgment or the making of the order being appealed or, in matters heard by a referee, within 60 days after the order of the referee becomes final.
Petition For Extraordinary Writ – Juvenile Dependency – JV-825
This form is filed in the Court of Appeal.
